We are an acute dialysis service. Patients come to us from admitting units for treatment. They are usually on several drugs, which we withhold for various reasons (BP, dialyzability) and our administration says we must call MDs for an order to with hold each med. Has anyone had to address this issue by creating standing orders or protocols for use in their hospital?
