I work in a Children's Dialysis Clinic. Most of the patients are teenagers and several have turned 18. Has anyone discussed advanced directives with young adults and what forms or material have you used?

Although these discussions are never easy, especially with people who are young, they have as much right as older people to share their wishes with their families and the healthcare team. One resource booklet that may complement this discussion is the "Advance Directives" publication by the National Kidney Foundation. You can find it in the A-Z Guide on the NKF web site at www.kidney.org. I hope that others will post other suggestions.
