I work in an acute facility that is currently providing hemodialysis for a 4 month old baby 4x's a week. The plan is to send the baby home hopefully in the next couple of weeks. After that the baby will come back to the hospital for dialysis until he is big enough to transplant. We are looking at about a year to a year and a half. Since I work acutes I'm not sure of the standards for checking pre/post bun, PTH, ect. What are the guidelines for checking these? Where should the PTH be maintained? How about the HGB? Where should the KT/V be? I want to set up a care plan that everyone in the Acute unit can follow. As an Acute Unit we don't use any of the chronic programs that track lab work, or remind of lab to drawn.
