What methods do you use to monitor clotting during dialysis of acute patients at intensive care with high bleeding risks under continuing heparin infusion? Do you measure ACT or APTT? How many times during session? What borders do you use to decide to higher or lower the heparin infusion or to leave it as it is?

We use Heparin 25000ky/Nacl 0,9% 100ml. During Hemoperfusion the recommended ACT-level is 300. Heparin infusion can vary between 1-40ml/h and you can also give 1-3ml doses in between. We use Heparin only during Hemoperfusions or occasionally in Plasmapheresis. In HD, HDF and UF we mainly use Fragmin.
