Fat in venous chamber

What is lipid build-up in the venous chamber indicative of? It's not the usual bulbous blood clots that occur often, but rather a layer of cholesterol(?) that forms at the top of the chamber prior to passing the air detector sensors. Flushing the line doesn't help nor does trying to aspirate the viscous matter out via syringe (as the matter is too thick to come up the med port tube)
