Recently I have had 2 art cath lines become loose at the hub during a run. I also had a line come apart when a restless patient jumped up out of his chair. The place I work wants to fire me for unsafe practice and inform the nsg board in my state. I have been working in dialsis for 18 months but I'm a RN with 25 years exper. These are the only disconnects I have had.But they all happen only days apart.Does the hubs ever fail? Do you get bad lots of tubing. I'm sure the cath hubs were finger tight.

I need a little more information to answer your question. First was it a disconnect at the female to male luer connector? The first response in a manufacturing setting is to blame the person. Deming showed that it is rarely the person at fault. It is usually the process. Post your e mail and I will contact you directly.
