Does anyone have a practice of decreasing pump speeds with someone with cardiac issues? (or evidence-based information)

I have no evidence for this, but our most symptomatic dialysis patient has less chest pain with blood flow below 250mls/min.

Try using a Step UF Profile with very minimum pulling for the first 15 min of each hour. This will allow cardiac refill and the patient will tolerate the treatment better. I tried it and it works for our cardiac patients.
