For anyone who adds these things to their acids, what documentation or labeling do you do? Is there a standard?

The Physician order is verified and signed off on the treatment record. The additives from Edlaw have labels that you stick to the jug and mark what the starting concentration was and what it was adjusted to, along with a line for the nurse to sign on.

Just to add a little to Chuck's response...

Edlaw provides, with every order, Concentrate Adjustment Labels that you affix to the concentrate container. As Chuck said, space is provided for gram weight added, date, time and signature.

We also provide Additive Adjustment Slide Guides and wall charts (which show the exact gram weight needed to raise concentrate 0.5 to 2 mEq's)and Dialysate Additive Mixing Guidelines.
