If crit-line is clea exempt, and the dialysis unit is a hospital based out pt facility, dose the crit-line fall under lab standards or Hospital standards as far as Joint commission is concerned? Has anyone encountered this?

The Crit-Line would fall under lab standards under the hospital umbrella(assuming that the hospital owns the clinic). Being CLIA Exempt means that you do not have to have a license to operate as a laboratory, nor do you need the waiver.
