Has anyone had a patient develop methemoglobinemia during dialysis not related to reverse osmosis malfunction?

Hi:

Have not heard or read of condition brought on by reverse osmosis malfunction. Perhaps others have.
However, can't this condition be inherited?

Methemoglobin can occur with high levels of chloramines and/or nitrates. This patient has no hx of methemoglobin. We think he developed it during his dialysis treatment.
