There are a number of comprehensive programs on the market, which can cost six figures and above. MIQS, AMI, Velos, Clinical Computing are examples of excellent programs. Most are "patient centric" requiring input in all care settings of the patient, so that everything is tracked. We've developed a simple, inexpensive database that tracks the key outcomes based on targeted parameters set up in advance with the Medical Director. For example, if a target is to have 80% of the patients with Kt/V of 1.3 or greater, this will track it, and identify the patients who fall out of that parameter. We've also developed a patient risk assessment tool which interfaces with a long-term care plan, and short-term care plan. The key is to carefully identify what your needs are so that you can find the best program to fit those needs.
