Question:
If you are an INDEPENDENT DIALYSIS FACILITY (not hospital-based), do you typically contract with your managed care and commercial insurances for dialysis coverage, or do you stay non-contracted/out of network (OON) and make arrangements on a case-by-case basis?

Choices:
We contract with all of our payors and do not accept out of network patients
We contract with most of our payors and negotiate rates for occasional out of network patients
We are non-contracted/OON for 1-25% of our patients
We are non-contracted/OON for 26-50% of our patients
We stay non-contracted/OON for >50% of our patients
