Does anyone add Vancomycin to the heparin flush solution for suspected catheter infections? Our clinic is thinking about initiating this soon. The policies I have read are a little vague. Can anyone tell me how they do this. (How much vanco to how much heparin and how long?) Also does this seem to work? Thanks
