Can you refer me to someone familiar with billing issues (CPT codes, etc) for physician orders which read venous mapping bilateral upper extremities with diagnosis of ESRD. Thanks for you help.

Maybe this will help. Dr. Jack Work, intervential nephrologist at Emory University in Atlanta, told me we should code all mapping studies for dialysis patients as:
Multiple Venous Trauma. These patients have had numerous venous punctures and other things, therefore, multiple venous trauma is appropriate.
Maybe that's not quite your answer since I am not a coder, but maybe it will help.
