Does anyone know where I can get my hands on a access surveillance tool/critical pathway?? I keep hearing/reading about them and have never seen one. I need to start using one but there is just so much information needed on it I don't know where to start. I would rather not re-invent the wheel if someone has one that they would be willing to share with me.

Medisystems has Access Alert, available from Henry Schein at 1-800-521-1472. The device fits onto the fistula needle prior to cannulation. Internal access pressures are obtained from both the arterial and venous needles. This is an easy form of trend analysis that takes very little time and is relatively inexpensive. Medisystems will send you articles, a laminated procedure card, etc. They can be contacted at 1-800-369-6334.

I don't think a device is what teeree2 is asking for. She is looking for a "form" that helps in tracking the access progressions, revisions, etc.

literally 1000's of dialysis centers utilized the Transonics flow device to monitor flow, recirc. and cardiac output. It can be used at ANY time during the dialyis session. No possibility of human error as all measurements are done on the tubing. No disposables to buy.
