Take a look at the inlet water pressure regulator. I have seen the o-ring in the regulator fail causing too high water pressure. This will make a heat exchanger leak. I am not sure why at 3000 hours this would fail but it is worth a try. Let us know.

JP is dead on. Many times if the inlet pressure is too high it will make them leak. Also they are just junk.

HI~
1) I'll like to thank's to (JP and XxROOKIExX) for the comments posted.
Problem it's been fixed...!!!!
this dialysis unit has (more than 25 years old ) after several machines leak ,i found that for some reason distribution loop doesn't had pressure relif valve (causing pressure to increase 30 to 40 psi when dialysis machine, renatron's,bicarb mixer, and acid mixer were off at night.)

solution: add pressure relief valve to distribution loop , after distribution pump, and before ,ultrafilter's; with a returning line to storage tank from pressure relif valve.( system disinfected after all done)

once again thank you JP and XxROKIExX FOR YOUR HELP!!!
Marco
