Hello, my center recently upgraded the ro (water system) because of a new addition. The new add. and the existing run on the same water loop. Well the culture results have been taken to send for analysis but have not been sent and we were given the go ahead to dialyze patients. I am the biomed tech and responsible for the machines and the water system. I feel that this is not safe but don't know who to report to or even if i should. THanks for your time.!!

You don't give enough information for anyone to make a reasonable assumption as to whether there is any problem.

You say the system was up-graded due to "an addition" (of stations I guess). Usually this is a matter of adding RO membranes and (if needed) increasing capacity of / or replacing the pre-treatment devices (carbons, softener) and possibly other pre-treatment changes.

Here's a few questions:

what devices exactly were upgraded?

do you have ultarfiltration or other sub-micronic final filtration?

did any of the work involve the storage tank and loop pumps (if present) or loop piping?

was the loop sanitized after the upgrade?

does the loop have a history of elevated culture / endotoxin levels?

You say samples wre drawn but were not sent -why was this?

It is standard protocol to draw validation chemistry and microbiological samples and have results prior to use of a new system.
And certainly after any changes to the water system both chemical analysis and microbiological sampling should be done however - the nature of an upgrade and other conditions may preclude the need to wait for results before using the system.

Before making an assumption be sure you know the circumstances, and if you feel strongly that proper procedure was not followed then you should speak with the Medical Director of your facility first, as by law he or she is the person responsible for the delivery of care to your patients.
