I thought it might be fun starting this topic since I just found out that I screwed one of the back panel screws right throught the level detector ribbon cable.
I've done an annual PM on a machine that I had just done the prior month. Now I put my glasses on to read the PM schedule! No wonder everything was dead nuts on.

Our piped pure water loop used to have manual ball valves at each station. I removed the machine water line without first turning off the ball valve. Water pressure in the loop was about 50-60 PSI. Ball valves were about groin height. Water stream hit me right in the you know what. It hurt!!! When I recovered and turned around it looked like I peed my pants. Would have been alright except for the 20 or so witnesses.
Where would I start. How about something similar to the last but numerous times mostly when I first started in dialysis. Good thing scrubs dry quick. But the one that made me almost go completely mad was trying to do the P2 test on phoenix but was using and external meter to get my pressures, machine was just seeing these insane pressures and shutting down the flow path. Had a few people stumped even from Gambro tech support, checked a handful of other things and finally got someone on the phone that asked me about the test setup. When I was asked why I was using an external meter it finally rang the right bell.
About 11 yearsa ago, I was working on an "E" machine. I was in a clinic that would experience numerous breakdowns per day. There were 4 machines down and the tech room was quite messy. Long story short, after about 4-6 hours of trying to troubleshoot a flow error, I decided to call it a day and try with fresh eyes tomorrow. I went to disconnect the machine, only to find out that it was never actually hooked up to the water.

don't know how many times I put a check valve in the wrong direction during a C3 pm... REALLY stupid, doing 'check AC connections' on K pm poking around in the power supply and after I finish, look over and its still PLUGGED IN and switch is ON!!! Lord was lookin out for me that day.... or maybe He was lookin for me..

After doing an annual pm on a 2008H, I forgot to hook up the outlet side of the UF pump. I put the machine in rinse with the hydraulics hanging out the back and when to lunch. When I came back I had burned up the blood pump board and M400 blood presser module. I never told the chief Technician what happened.
This is kind of embarassing for me to admit, but what the heck. I've been in dialysis as a biomed for 15 years now, so I should know better. One day last year, I think in July, it was a really nice day I remember. I was a minute late for work. I was so embarassed. I know I'm not perfect but to make my first mistake like that is just crazy.

Sounds like chiefa needs the shoes checked on his high horse.
I've done several CIP's on our RO. One time after dwelling the Puristeril I turned on the RO to rinse it out. I didn't realize I hadn't closed the disinfect port with line to drain. When the pump kick on, lets just say that the hose took off like a whipping snake drenching everything including myself. I hit the drench shower and had a mess to clean up. Fun Day!!

Not that long ago I was rebeding a carbon tank. Closed the valves and started to take off the front of the head to replace piston and rings. Only thing is I forgot to bleed the pressure out of the lines or the tank. That thing came flying at me like when my mom use to whoop me. Was soaking wet all day long.

I got one better than Chiefa. I have never made a mistake in all my 20+ yrs in dialysis
We had Echo machines for reuse and used home-made 14 gallon tanks to fill with 1.5% or 3% Renalin. I turned the water valve on to fill one of the tanks and charge nurse said you need to walk over to the hospital and deliver these stat labs (about 1/4 mi away and on 4th floor). I was handing the labs over to the hospital when the light bulb went off. Fastest run ever back to close that water valve. When I opened the door there was the water!

Its funny because the people who say they never make a mistake are probably the ones who always blame it on someone else. So in their mind, they are perfect...
