I am writing a procedure in the event of chlorine breakthrough at our second carbon tank for our 24 station facility. So far I have stated that dialysis must be terminated but what should we do with patients blood ? Return it or not? Any opinions out there

That is up to your Medical Director. Some will allow you to continue and use the water in the storage tank as long as it is negative for chlorine (turn off RO of course) and others will have you discontinue and not return the blood. You need to make your Medical Director aware of the different options and how you test so he/she can make a good decision.
