Have you check the cond. cell calibration with external meter?
You did not mention checking the balancing chamber volume. What is it set at now? Most K machines run around 61.3 ML. There may be a problem with the BC membranes, or a valve. Click on the "Find" button at the top of the screen here and search balancing chamber, or membrane for several ways to check this out. Did the independent meter indicate that the conductivity was the same as what the machine said? To keep this really simple, are you sure that the acid and bicarb that you are using is the correct mix and family and not contaminated? And are the NA and bicarb settings what they are supposed to be for the type of concentrate that you are using? When was the machine made and how many hours are on it?

What is the base sodium and bicarb set for and what does your TCD box say the condo should be? Have you recently rebuilt your conc pumps? I have learned that after rebuilding pumps not to calibrate them unless you are not getting good condo. If you have a good final condo that matches your meter, don't mess with measuring the pump's output. Also it's hard to believe that at 6000 hrs you have a torn BC membrane. But that's easy enough to check by putting the hansens in acid concentrate.

turns out all that needed done was bc volume being lowered
